The offer covers only the services listed; any additional treatments, imaging, or follow-up visits may incur additional charges, which will be discussed with you in advance.
We reserve the right to decline or reschedule appointments when clinically or operationally necessary.
4. Appointments, Cancellations & No-Shows
When you submit a form, our team will contact you to confirm your appointment date and time.
If you need to cancel or reschedule, please notify us as soon as possible (ideally at least 24 hours in advance).
Repeated last-minute cancellations or no-shows may result in limitations on future appointment scheduling or require a deposit.
5. Payments, Insurance & Financial Responsibility
Payment policies can vary by service and by patient.
Some services may be self-pay and not billed to insurance.
If insurance is used, coverage and benefits are determined by your insurance carrier. You are responsible for any copays, deductibles, or non-covered services.
